  When you mail these items to us, Include:

  Only one Parking Card/ 'decal' will be issued per person and may be transferred between vehicles transporting the handicapped person.

  The handicap parking Card/ 'decal' is only valid at U.S. Military Facilities in the European Medical Command footprint and only for the eligible dates specified on the form.

  Parking Card will be mailed to your current APO address.
